New Patients (Initial Evaluations & Consultation Evaluations): After booking, you’ll receive your intake paperwork and appointment details through Jane. Please complete all required forms prior to your appointment.

Returning Patients (Follow-Up Visits): Simply book the follow-up service that best fits your needs. If your condition has changed significantly or you’re unsure whether to schedule a follow-up or a new evaluation, please reach out before booking.

My name is Daria Tavana. I am a board-certified physical therapist who specializes in pelvic health, lymphedema and general orthopedic rehabilitation. I graduated from Columbia University Medical Center in 2017 and started my journey into women’s health in 2021.
